In this review, we describe fundamental aspects of the hepatic lymphatic system and its role in a range of liver-related pathological conditions such as portal hypertension, ascites formation, malignant tumours, liver transplantation, congenital liver diseases, non-alcoholic fatty liver disease, and hepatic encephalopathy.

There are three categories of liver lymphatic vessels characterized by location: capsular, sublobular, and portal. Liver lymph is generated from hepatic sinusoids in the space of Disse and is connected with the peri-portal space of Mall where the lymphatic capillary vessels begin .

In normal conditions, hepatic lymphatics are not depicted on cross-sectional imaging. They are divided in lymphatics of deep system (lymphatics following the hepatic veins and the portal tract) and those of superficial system (convex surface and inferior surface).

The Hepatic Portal System is defined as a unique circulatory system that connects the capillary beds of the liver and the small intestine/spleen, allowing for the mixing of deoxygenated and oxygenated blood before draining into the hepatic vein.
