Spy knife is not effected by damage boosts (Legendary hero, flex, etc.). It is however affected by Dull Knife's Defense downs as they nerf the defense of the Sans instead of making you deal extra damage.

This will also unlock Dull Knife, a skin for the Worn Dagger. You can always return to Whitespace through recompleting the REPRESSED achievement or by petting Mewo in the Casino, which will also take you to Whitespace.

(Example: Dull Knife is swung in Sad mode, right before it hits the Sans switch to angry mode. It will take angry mode's damage and stamina drain instead of Sad mode's.) This technique is really useful to deal high damage without losing all your health.
