HFS, also known as palmar-plantar erythrodysesthesia syndrome, acral erythema, Burgdorf’s syndrome, and more recently grouped with the so-called toxic erythema of chemotherapy syndromes, is a relatively common skin reaction to chemotherapy.

It may also occur with EGFR / protein kinase inhibitors and BRAFinhibitors such as vemurafenib and dabrafenib; these also cause palmoplantar keratoderma (thickened skin). The most commonly used drugs that cause hand-foot syndrome are intravenous 5-fluorouracil and its prodrug oral capecitabine.

Chemotherapy-induced acral erythema, also known as palmar-plantar erythrodysesthesia or hand-foot syndrome is reddening, swelling, numbness and desquamation (skin sloughing or peeling) on palms of the hands and soles of the feet (and, occasionally, on the knees, elbows, and elsewhere) that can occur after chemotherapy in patients with cancer.

HFS, also called palmar-plantar erythrodysesthesia, is a distinctive and relatively frequent dermatological toxic reaction associated with certain chemotherapeutic agents, specifically capecitabine, infusional fluorouracil and liposomal doxorubicin (2). The manifestations of HFS have been classified into 3 grades according to their severity (3).

Palmar-Plantar Erythrodysaesthesia (PPE) or Hand Foot Syndrome Symptoms include: redness, swelling, tingling or burning sensation, tenderness, sensitivity to touch, tightness of the skin, thick calluses or blisters on the palms of hands and/or soles of feet, cracking, flaking or peeling of the skin, soreness or severe pain.
