Noninfectious reaction to the materials used in the surgery is another consideration. Factitious fever is another diagnostic challenge. Any history of psychiatric illness should raise suspicion.

Among the other main infectious etiologies are perinephric abscesses, malaria, enteric fever, Epstein–Barr virus mononucleosis, cytomegalovirus and visceral leishmaniasis (kala azar).
